Letter of Inquiry (LOI) Instructions
1. LOI Cover Sheet
Please complete the Foundation's LOI cover sheet (Word® Doc, 32.5Kb). If you need assistance obtaining this cover sheet, call (650) 736-0676.

2. Three-Page Narrative
Please write a narrative that is no more than three
8 1/2" by 11" pages, single-spaced, non-reduced
type. We ask that in developing your narrative you use the
following format:
- Program Description: Briefly describe what you propose
to do and how you propose to do it, including information about the
timing, frequency, and duration of the proposed activities (e.g., will
provide one three-hour parent education workshop per week over a 16-week
period). Please include the need or opportunity being addressed; population
and geographic area to be served; and list preliminary measurable goals
and objectives.
- Project Budget: Please include the amount of funding
requested; how the funds will be used (principle costs); and other sources
of funding, both secured and potential. If you are requesting partial
funding for a program, also provide the total program budget.
- Organization: Background and Qualifications. Please
include your mission statement, primary services provided, geographic
area and population served (if different than that served by the proposed
program). Briefly explain why your organization is well suited to carry
out the proposed program.
3. Board of Directors Information
In addition to the LOI cover sheet and three-page narrative, please include a list of your organization's Board of Directors, with their contact information.
4. Submit Your Letter of Inquiry (items 1-3 above)
- By e-mail: Send an e-mail to grants@lpfch.org with the completed LOI cover sheet, three-page narrative, and Board of Directors information attached. Please send these items as e-mail attachments, rather than in the body of an e-mail.

Please note that incomplete LOIs or those that do not follow the prescribed format may delay the review process and result in the LOI being held over to subsequent funding cycles.

- Three-Page Narrative
- Board of Directors Information
