Why It's Important

Care coordination addresses the interrelated medical, social, developmental, behavioral, educational, and financial needs of families. Effective pediatric care coordination provides benefits across all levels of the health care system, from improving delivery of services and decreasing family burden to reducing health care costs and utilization.

Our Approach

We support effective care coordination that connects children to services, facilitates provider communication, and supports families as primary caregivers. We want all families to have access to effective and appropriate care coordination services and supports. Our priorities include:

  • Developing national standards for care coordination systems
  • Documenting care coordination issues and system barriers at the state level
  • Supporting county-level innovations to improve inter-agency care coordination
  • Funding research on best practices for care coordination
