Dear Network Member:

An August 2012 survey of Network members identified care coordination as one of the most pressing challenges in caring for children with special health care needs. To understand the perceptions and experiences of Network members, we have developed a brief survey on care coordination strategies and practices. The survey contains seven questions and should take three to five minutes to complete:

The Foundation currently is pursuing several approaches to help improve care coordination for children, their families, and providers. We recently announced several grants related to care coordination. These include commissioned research with Health Management Associates to review policies and practices related to care coordination, and grants in Kern and Fresno counties to expand or replicate existing care coordination models for children with special health care needs. Last week, the Foundation published a Request for Proposals for projects from community-based, multi-agency coalitions interested in participating in the California Community Care Coordination Collaborative.

The findings from this survey will expand our knowledge and help inform future projects and advocacy efforts. As always, we will publish the findings from the survey and forward them to the Network. Thank you for taking a few minutes to offer your perspective.
