Purpose
To evaluate and replicate an existing model of care coordination for young CSHCN in at least three California counties.
Outcome
This project was intended to replicate an existing model for interagency collaboration to improve care coordination for children with special health care needs to three new counties in California. The project successfully engaged leaders in Contra Costa, Monterey and Orange counties to establish care coordination coalitions. The coalitions in each county meet monthly and each were able to support a system-level care coordinator to begin monitoring their impact.
