Please read this document and all attachments, and submit requested materials in the required format. We are happy to assist you with any questions that you may have about the proposal process. Submitting Your Proposal
Please include your organization name and proposal title in ALL correspondence with the Foundation (including e-mails). Please date proposal materials, and send all components of your proposal in ONE e-mail to firstname.lastname@example.org. Forms requiring a signature must be signed, scanned, and included in the e-mail.
Proposal Review Process
The proposal review phase is an interactive process between the applicant and Foundation staff, and may include requests for additional information. Foundation staff also may seek information from individuals and/or organizations that are likely to be familiar with your work. Recommendations by Foundation staff will be presented to the Foundation’s Board of Directors, which is responsible for making funding decisions.
The proposal should be in Times New Roman, Calibri, or Arial font, size 11, with 1 inch margins. It may be either single or 1.5 spaced, and with numbered pages.
PLEASE PROVIDE THE FOLLOWING PROPOSAL SECTIONS:
I. Cover Letter
Provide a signed letter to the Foundation from the project director on the applicant institution’s letterhead requesting support for the project.
II. Organization and Project Information Form
Complete the attached form "B. Organization and Project Information Form" (Word).
III. Proposal Narrative (up to 15 pages total, excluding CVs and/or resumes, and also excluding sections IV, V, and VI)
1. Executive Summary
Provide a brief (<250 words) summary of the proposed project including the issue that will be addressed, the project’s principal activities, the target audience for the project’s outcomes, the major products of the grant, and methods by which the outcomes will be disseminated
a. Describe the nature and the importance of the issue your project is intended to address.
b. Describe previous efforts by your organization and others to address this issue and how your project will build on those efforts and the lessons they offer.
c. Describe the goals of the proposed project and how the project will contribute to solving or ameliorating the identified issue.
d. Describe the difference this project will make for children. Briefly, explain how the project will contribute to improvements in systems of care and the health and well-being of children, or how it will raise public or professional awareness of children’s health issues.
a. Describe in detail how the work you propose will be done, including the specific results you hope to achieve and why this approach was chosen. Include a work plan/timeline that indicates major activities, stages of work, key events, products, data collection or sources, and analytic methods, if appropriate. For those who are applying for a grant to address issues related to CSHCN, please include a brief (1-2 sentences) definition of CSHCN as it is being applied to your proposed project. If you are using the federal MCHB definition, you may simply reference that definition and not restate it in your proposal.
b. Discuss possible barriers to the success of the project and potential limitations of the proposed work in addressing the identified issue.
c. If consultants or subcontractors are employed, specify the work for which they will be responsible and how that work will be carried out.
d. If the project will utilize an advisory committee, describe its functions and likely composition.
a. Provide a list or table of specific products that will result from this project, including proposed dates of completion.
b. Describe proposed plans for disseminating the products or lessons from this project, including the intended audience(s); consider dissemination methods beyond the production of publishable material.
c. Describe any anticipated subsequent related work if applicable.
5. Grantee Organization
a. Briefly describe the mission, size, length of time in existence, assets, major sources of support, and organizational structure/administration of the grantee organization that will carry out the project.
b. Describe the qualifications and capacity of any collaborating organizations, subcontractors, and consultants.
6. Project Management
Describe the Project Director, and other key staff (e.g. degrees, experience in the field, unique qualifications including related past work). Please also attach curriculum vitae or resumes for the Project Director and other key staff listed in the project budget (the CVs and/or resumes do not count toward the total 15 page limit).
7. Budget Justification
Provide a brief justification for each budget line, including brief job descriptions for unfilled positions. Please refer to "D. Budget Guidelines."

V. Letters of Support and Commitment
a. Any work to be done by a consultant or subcontractor should be described in a letter of commitment that includes the scope of work, budget with justification, timeframe, work plan, and resumes.
b. Letters of support from organizations external to the grantee may be provided but should detail the specific contribution to be offered by the supporting organizations.
VI. Tax Papers
Provide evidence that the grantee institution is exempt from federal taxation under IRS Code 501(c)(3) and not a private foundation under IRS Code 509(a), along with a letter signed by a responsible officer of the applicant institution certifying that the copies are true and correct copies of the originals on file and that they remain in full force and effect. If the grantee organization is not exempt from federal taxation under IRS Code 501(c)(3), please provide the organization’s articles of incorporation and the organization’s most recent audited financial statements.
