A continuation of the previous project Improving the Readiness of Transitioning CCS Adolescents: Building a Best Practice Model. To ensure a sample size that is representative and generalizable, researchers will attempt to collect more outcome data on the transition process from pediatric to adult health care for children in the CCS program in Kern County.
