Addressing the alarming lack of transportation to specialty care became a top priority for the San Joaquin County Community Care Coordination Collaborative (SJ 5Cs), a coalition led by the Family Resource Network that brings together representatives from a wide range of children’s services agencies. The SJ 5Cs collaborated with the San Joaquin County Regional Transit District to launch a transportation service to take patients from their homes to their approved providers in Sacramento and the Greater San Francisco Bay Area. The development of RTD Care Connection is an example of how a coalition that connects with committed partners and forges new relationships can effect systems change.
